The Family Home Program provides an accommodation option for children and adults with an intellectual disability who for a variety of reasons cannot or choose not to live with their family. The Family Home Program is designed to enable a person to live as an integral member of an alternative family setting and is supported by an approved Family Home Provider. Wherever possible, the person\u2019s relationship with his-her natural family is encouraged and supported by the program. The program takes a person-centered approach to providing supports and services that allow for the attainment of Personal Outcome Measures. All placements are monitored and supervised by a social worker to ensure the needs of the person are being met. The Family Home Provider receives on-going training and support from the program social worker. Criteria, Children and youth under the age of 18 who have a primary diagnosis of an intellectual disability are eligible to receive services from the children\u2019s Family Home Program. Parents of children and youth must be living within Simcoe County.
