Case Management @ York Support Services Network

​York Support Services Network (YSSN) is a non‐profit, community‐based agency that provides case management services to children and adults who have developmental disabilities living in York Region. Case management workers help individuals and their families to plan for the services needed to support the individual with a disability as well as connect them with services available in the community. The individual or family may wish to be connected to services such as, – Respite-camps – Special education services – Government funding resources – Health services – Adult services
